Pertussis: Always on the agenda

Regarding the fact that pertussis disease remains as a serious public health problem and vaccination is the only preventing route to combat this disease, Bordetella pertussis and pertussis vaccines have continued to receive worldwide interest. At present, the changes in the patterns of infection of vaccinated individuals as well as susceptible age groups are found to be worthy of detailed evaluation. Because pertussis is a major cause of both childhood illness and infant death and is also reported among adolescents and adults, it is clear that the pathogen keeps on circulating in populations. Currently, universal adolescent and adult vaccination with the new acellular pertussis vaccine has important impact on the world's agenda. However, this type of vaccination is in use for primary immunization in a very limited number of countries and it can hardly enter to poor and developing countries due to its high cost. In addition to this dilemma, the intrinsic variations among such vaccines in use points to the need for further stuides in the field. In this report, the current pertussis outbreaks, the virulance factors of B. pertussis, the genetic variation that the organism displays and pertussis vaccines are reviewed with particular reference to the interactions amongst them.
