The Comparison of In-Vitro Activity of Doripenem and Other Carbapenems on Gram Negative Bacteria Isolated From Clinical Samples

Objective: In this study, it was aimed to compare the activity of doripenem, one of the antibiotics recently used in clinics in our country, and other carbapenems on gram negative bacteria under in-vitro conditions.

Material and Methods: Doripenem, imipenem, meropenem and ertapenem susceptibilities of 70 Escherichia coli, 36 Enterobacter spp, 71 Klebsiella pneumoniae and 71 Pseudomonas aeruginosa strains isolated from various clinical samples and identified with conventional methods were investigated with Kirby-Bauer disk diffusion method according to the Clinical and Laboratory Standards Institute (CLSI) criteria by Bioanalyse© antibiotic disks.

Results: We examined totally 248 clinical samples (143 urine, 48 wound site, 33 blood, 21 ear discharge and 3 tracheal aspirates) in our study. One hundred and twenty-nine (52.0%) of the total amount of specimens were obtained from inpatients and 119 (48.0%) from outpatients and it was deduced that 142 (57.3%) of the bacterial strains were community-based and 106 strains (42.7%) hospital-acquired. All of the E. coli and Enterobacter spp were susceptible to carbapenems. On the other hand, doripenem and meropenem were detected to have the lowest resistance rates for K. pneumoniae and doripenem for P. aeruginosa among carbapenems.

Conclusion: Carbapenem resistance of P. aeruginosa strains was detected to be higher when compared to E. coli, Enterobacter spp and K. pneumoniae strains, in our region. Taking into account all the bacteria in our study, it was detected that the rates of doripenem resistance were nearly the same as the rates of resistance to other carbapenems.
