| A Rare Case: Urinary Tract Infection Due to Lactobacillus delbrueckii |

Urinary tract infections (UTIs) are common bacterial infections in which the causative organism is mostly
uropathogenic Escherichia coli. Lactobacilli, which are usually considered as contaminants, have been
associated with some infections in recent years. In this case report, we present the first case of UTI caused
by Lactobacillus delbrueckii (L. delbrueckii) from Türkiye. The urine culture of a 91-year-old woman with
complaints of pain during urination, burning, frequent urination, nocturia and haematuria showed alpha and
non-haemolytic, gram positive thick bacilli and clustered cocci. The predominant gram positive cocci were
identified as Staphylococcus hominis (S. hominis) by MALDI-TOF MS and considered as contamination, but
empirical treatment (nitrofurantoin plus fosfomycin) was started due to clinical findings. Two weeks later, the
patient was admitted to hospital again with the same complaints. In repeated urine cultures, the gram-positive
bacilli grown >105 CFU/mL pure on blood agar were identified as L. delbrueckii by MALDI-TOF MS. According
to the antibiotic susceptibility test result by E test, the patient’s clinical condition improved completely with
15-day amoxicillin treatment. The fact that the patient consumed yoghurt and kefir every day strengthened the
possibility of urinary tract infection after vaginal colonisation with L. delbrueckii. In conclusion, it is thought that
a significant proportion of cases may be overlooked since L. delbrueckii is considered commensal. This first case
of L. delbrueckii UTI reported from Türrkiye showed that L. delbrueckii should be kept in mind as a possible UTI
agent especially in elderly women consuming probiotics.
