A Rare Case of Perinephritic Abscess Due to Group A Beta Haemolytic Streptococci |

Group A beta hemolytic streptoccocci may lead to various diseases such as acute tonsillo pharyngitis, cellulitis,
scarlet fever and acute rheumatic fever. Perinephritic abscesses develop either as a result of ascending pyelonephritis
or in the setting of bacteremia with hemotegenous spread. The most common causative agent of pyelonephritic
abscess is Escherichia coli ascending to kidney tissue. The development of pyelonephritic absecess due to group
A beta hemolytic streptococci are rarely seen. In this report, a case in whom pyogenic abscess developed due
to group A beta hemolytic streptococci was presented. Abdominal ultrasonographic examination of a 53 years
old male patient who referred with the complaint of left flank pain, revealed the presence of a perinephritic
abscess. Abscess formation with air density extending up to crista iliaca along iliopsoas border was observed in
perirenal area. Group A beta hemolytic streptoccoccus was isolated in the abscess drainage material. Amoxycillin
clavulanic acid was initiated and continued for three weeks. Following antibiotherapy atrophic left kidney was
removed. Different pathogens can lead to the development of perinephritic abscess, thus it is important to identify
the causative agent and start appropriate therapy.
