In-vitro Activity of Daptomycin Against Enterococcus strains Isolated from Blood Cultures

Objective: In recent years, increasing antimicrobial resistance in Enterococcus strains cause important problems especially in nosocomial infection all over the world. The effectiveness of daptomycin which is a cell wall-inhibiting agent with a spectrum of activity limited to gram-positive bacteria, has been demonstrated to have activity also against resistant enterococcus strains. The aim of this study is to identify the species and determine daptomycin susceptibility of enterococci isolated from blood cultures.

Materials and Methods: In our study, a total of 64 Enterococcus strains isolated from blood culture samples of hospitalized patients between May 2011 to January 2014, were evaluated. The strains were identified at species level by API-20 STREP (BioMérieux, France) and/or automated VITEK 2 system (BioMérieux, France). Daptomycin susceptibilities were determined by E-test (AB Biodisk, Solna, Sveden) according to the CLSI (Clinical and Laboratory Standards Institute) guidelines. Suscepti-bilities to the other antibiotics were determined by Kirby-Bauer disc diffusion method according to CLSI guidelines.

Results: Sixty-four strains isolated from blood cultures of different patients were identified at species level. Forty (62.5%) strains were identified as Enterococcus faecium, and 24 (37.5%) as Enterococcus faecalis. All of the isolates were susceptible to daptomycin and MIC range was 0.16-4 μg/ml. MIC50 and MIC90 values for E. faecium, and E. faecalis were 1.5-3 μg/ml, and 0.19-1 μg/m, respectively. Three of the E. faecium isolates were resistant to vancomycin. Daptomycin MIC values for these two strains were 3 μg/ml and 2 μg/ml, respectively.

Conclusion: Daptomycin exhibited in-vitro effectiveness against enterococci including vancomycin-resistant enterocci strains. Daptomycin may be thought to be a good alternative for the treatment of resistant enterococcal infections.
