Fusarium Keratitis Progressing to Endophtalmitis After Corn Tassel Contact: Case Report

Fusarium is a genus of mold fungi that is usually found in the soil and on plants. Fusarium keratitis may permeate rapidly with the facilitating effect of various risk factors and corneal destruction and endophthalmitis may develop eventually. In this report, we present a 50-year-old male patient, whose left eye was traumatized by a corn tassel while working in the farm and applied to an ophthalmology clinic with complaints of pain and decreased vision. As his complaints continued despite treatment, he was referred to ophthalmology clinic of a university hospital. Corneal opacity and conjunctival hyperemia were observed in the left eye. Possibility of a fungal infection was considered due to the presence of a 5x5 mm epithelial defect and a white stromal infiltrate with feathery edges in the inferior paracentral part of the cornea during biomicroscopic examination. Mold colonies grew in culture and were interpreted as Fusarium spp. with lactophenol cotton blue staining. The isolated strain was identified by sequence analysis, and the causative agent was determined as ‘Fusarium solani species complex’. Despite medical treatment and surgical operations, the infection spread to the intraocular structures, which obliged the evisceration of the eye. No signs of infection were found in the postoperative follow-up examinations. It is thought that this case report may contribute to the literature in terms of drawing attention to the transmission routes and clinical course of the causative agent.
