Usage of polymerase chain reaction in the diagnosis of tuberculous meningitis

The early diagnosis of tuberculous meningitis (TBM) is very important. In this study, the value of the polymerase chain reaction (PCR), one of the DNA-based assays, was compared together with conventional methods [Ehrlich-Ziehl-Neelsen (EZN) and culture] for the detection of Mycobacterium tuberculosis in cerebrospinal fluid (CSF) from patients suspected of TBM. Of the 68 CSF specimens from probable TBM patients, 7 were positive by PCR, 5 were positive by culture (BACTEC 460/ Löwenstein-Jensen). No positive results were found by EZN. The PCR for this study had a sensitivity of 80%, a specificity of 95%, a positive predictive value of 57%, and negative predictive value of 98%. In the diagnosis of TBM, PCR should be used together with conventional culture methods.
