The early diagnosis of tuberculous meningitis (TBM) is very important. In this study, the value of the polymerase chain
reaction (PCR), one of the DNA-based assays, was compared together with conventional methods [Ehrlich-Ziehl-Neelsen
(EZN) and culture] for the detection of Mycobacterium tuberculosis in cerebrospinal fluid (CSF) from patients suspected of
TBM. Of the 68 CSF specimens from probable TBM patients, 7 were positive by PCR, 5 were positive by culture (BACTEC
460/ Löwenstein-Jensen). No positive results were found by EZN. The PCR for this study had a sensitivity of 80%, a specificity
of 95%, a positive predictive value of 57%, and negative predictive value of 98%. In the diagnosis of TBM, PCR should be
used together with conventional culture methods.
